The First Assistant Role Surgical assistants are trained to perform routine surgical duties during a procedure. This includes preparing the operating room, positioning patients on the operating table and handing instruments or supplies to the surgeon as needed. The first assistant's role is to provide the surgeon with an extra pair of skilled hands at the site of the procedure. First assistants can make incisions and retract tissues to bare the surgical area for the surgeon. They also provide hemostasis and suction as needed, to keep blood from obstructing the surgeon's view. Once the surgeon's work is done, the first assistant can suture and close the incision, using drainage tubes or dressings when appropriate. Prerequisites Any candidate for recognition as a registered nurse first assistant must hold a valid nursing license and at least a bachelor's degree.

This can be a nursing degree or a baccalaureate in any other field. Nurses with an aptitude for surgical work begin as perioperative nurses, providing patient care before, during and after procedures. As they gain experience, they can advance into an assisting role, and then the first assistant role, by demonstrating the necessary skills.

Certification The Certified Registered Nurse First Assistant credential, or CRNFA, is administered by the Competency and Credentialing Institute. The questions cover both theoretical knowledge and skills, and the specific tasks a first assistant might be called upon to perform. Candidates who pass must maintain their certification through a five-year cycle of continuing education and recertification. National Board of Surgical Technology and Surgical Assisting A similar certification is available through the National Board of Surgical Technology and Surgical Assisting, or NBSTSA. Nurses who complete a surgical first assisting program accredited by the Commission on Accreditation for Allied Health Education Programs can apply for the NBSTSA's Certified Surgical First Assistant credential. The certification exam is similar in content to the CCI's examination, but consists of 200 questions rather than 150. Of those questions, 175 are scored and the remaining 25 are test questions under consideration for future exams. Certification must be renewed on a four-year cycle, either through continuing education or by passing a recertification exam.
